Applications are now available at our Referring Agencies.
If you would like assistance, please complete the application from a referring agency by October 26, 2024.
Eligibility Criteria:

Only parents or legal guardians are eligible to apply. We kindly ask that grandparents refrain from submitting applications to avoid duplicate requests for one child.

Eligible children include those from newborns to high school seniors and expecting mothers who are due before January 31, 2025.
